The Word documents below are templates of the Online RIG Ethics application forms. Students are advised to download the required template below before attempting the online version. Fill in the document and send to your Supervisor for review. Once supervisor is happy with the content, copy and paste from the Word document into the actual online RIG Ethics form.

RIG Registration Form
The RIG Registration Form should be filled in by the following categories of persons and e-mailed as indicated on the form.
- Persons external to UKZN that wish to use the RIG system to apply for ethical clearance. Login details will be provided by Research Office after details have been captured.
- Staff members who do not have a profile in the RIG system, but are involved as investigators or supervisors in a study that requires ethical clearance.
- Students who do not have a profile in the RIG system, but are involved as investigators in a study that requires ethical clearance.
For Publications: A researcher wishing to submit a publication or student supervision activity in the RIG system, but finds that an internal author involved in the research, or the student supervised, does not have a profile in the RIG system.
RIG Co-Investigators Declaration Form
Please note that it is a requirement that all co-investigators (internal and external to UKZN) in a study must complete the form in the link below.
After completion, this form must be given to the Principal Investigator of the Study. The Principal Investigator must upload the completed form on the attachments tab under Signatures.
